Dear Patients,

It is with great sadness that I am writing to inform you that after ten years at Community Health Network, I will be leaving to pursue a new journey within my career. My last day is Monday, April 3, 2023. It has been a privilege to work at Community Health Network, and I will miss you as one of the many patients I have cared for over the last several years. It is my hope that you will continue your care at Community Health Network where you may continue to have access to quality whole-person care. The following medical providers are available to accept you as your new primary care provider (PCP):
