A RECORD OF OUR SERVICE TO THE COMMUNITY
Stephen F. Austin Community Health Network, dba Community Health Network (CHN) was established in 2008 to provide accessible and affordable healthcare for people of Brazoria County. That year, the Center had one health care provider with three exam rooms providing care at the Adoue Family Health Center. Since that time, the Center has grown substantially, adding a dental department, women’s health care, behavioral health counselors, and psychiatry. In 2013, CHN launched a second site located in Freeport, Texas. The Freeport Community Health Center provides adult medicine, women’s health, obstetrics & gynecology, behavioral health, and general dentistry services.
Due to growth and demand for services in Alvin, CHN has opened four additional sites: the Women and Children’s Health Center, Callaway Behavioral Health Center where we provide psychiatry, group & individual counseling services; the Pearland Family Health Center; and the Children’s Clinic-League City. The Center’s newest location is our Mobile Clinic that is equipped with a medical and dental exam room. The Mobile Clinic will enable CHN to travel throughout the County delivering care in more remote areas in partnership with community partners. In the Center’s first year of service 1,025 patients received care, In 2017 the number of people that received the care was 13,350.

Stephen F. Austin Community Health Center, Inc., dba Community Health Network (CHN), receives HHS funding and has Federal Public Health Service (PHS) deemed status through the Federal Tort Claims Act (FTCA). As such, CHN is granted medical malpractice liability protection through the federal government acting as its primary insurer. As a deemed health center, CHN and its employees, working within the approved scope of project are considered federal employees and are immune from lawsuits for medical malpractice. A patient who alleges acts of medical malpractice by a deemed health center cannot sue the health center or the provider(s) directly, but must file the claim against the United States Government.

Individuals with family incomes below 200% of the federal poverty level are eligible for sliding scale fees. Medicaid, Medicare, CHIP, and most private insurances are accepted. Individuals who have no insurance and are eligible for discounted services may be placed in a discounted fee group based on their family size and household income.
Community Health Network does not turn away anyone away due to an inability to pay. We serve approximately 15,000 individuals per year and welcome anyone in need of health care services.
